1st is the most worn out part, usually. Grinding into first, popping out of gear, are all common issues. Often, its external stuff, but even once those things are addressed the parts for first are often already shot. They are old and suffer more wear than other parts of the transmission, even in a gear box that is maintained well. So, instead of pulling the transmission and doing the work, many wish to do it with the trans in the car. Its doable, as I showed, but not overly pleasant, as I also showed.
However, given the age of these cars, it is best policy to address the whole inside as it is highly likely that there are more bits needing addressed. Simply addressing 1st is just kicking the can down the road until 2nd, 3rd, etc start to fail.
